This manuscript presents a novel approach to targeting the TIGIT pathway in non-small cell lung cancer (NSCLC). While the methodology is robust, the current version lacks sufficient clarity in the "Comparison" group definition and requires significant refinement of the statistical power discussion before submission to a Tier A journal.
The P-value for the primary endpoint is reported as 0.042. Given the sample size (n=84), the power is borderline. I recommend adding a post-hoc power analysis or explicitly stating the limitations in the Discussion.
The novelty gap is clearly identified. The focus on TIGIT resistance in PD-L1 low-expressors is timely. However, the literature review (Introduction) misses the 2025 SKYSCRAPER-01 update. Including this will demonstrate up-to-date awareness of the competitive landscape.
Figure 2 (Kaplan-Meier curve) is cluttered. Suggest removing the 'Number at Risk' table from the plot area and placing it below the X-axis for better legibility.
The tone is generally appropriate, but there is excessive use of passive voice in the Results section. Recommendation: Change "It was observed that..." to "We observed..." to improve readability and engagement.
Check the formatting of references #12 and #18. They do not match the Vancouver style required by the target journal. Ethics approval number is missing from the Disclosure statement.

Study Design & PICO Audit
The Population (P) is well-defined. However, the Intervention (I) lacks detail regarding the titration phase. Specifically, the escalation protocol from 10mg to 30mg is not documented in the Methods section. This will be an immediate red flag for NEJM/Lancet editors.
